HealthTrust Coverage Plans

HealthTrust's goal is to provide more than competitive rates and comprehensive coverage options. We are committed to adding value, exceptional service, and expertise to a personal and valuable employee benefit. Our coverage options include medical, prescription drug, dental, short-term disability, long-term disability, and life.

HealthTrust medical plan options are provided in partnership with Anthem Blue Cross and Blue Shield (Anthem). This partnership means all HealthTrust medical plans include:

Vision Discount Program

Routine eye care is an important part of your overall health. HealthTrust partners with Anthem to offer a vision discount program to all Enrollees, Retirees, and their eligible family members who are enrolled in medical plan coverage. These discounts are available at thousands of eye care providers nationwide, including private practicing optometrists, ophthalmologists, opticians, and leading optical retailers such as Target Optical, LensCrafters, and Pearle Vision. Eligible HealthTrust participants can enjoy reduced fees for eye exams, contact lenses, eyeglass frames and lenses, and more.

Prescription Drug Coverage

HealthTrust medical plans include prescription drug coverage provided in partnership with CVS Caremark® or Anthem CarelonRx. 

CVS Caremark

CVS Caremark provides HealthTrust enrollees and their covered family members with cost-effective prescription drug benefits for both short- and long-term medication needs, all in an easy-to-use format. With CVS Caremark, HealthTrust Enrollees and their covered family members are part of one of the largest, most comprehensive coverage networks.

Anthem CarelonRx - Lumenos Preferred Blue, Access Blue New England and Open Access HDHP Plans

The prescription coverage offered with HealthTrust's High Deductible Health Plans is provided by CarelonRx, an Anthem company. CarelonRx prescription coverage includes: 

- Coverage for both short-term and long-term medication needs

- 24/7 access to the Pharmacy Member Services Team by calling 833.267.2133 

- The option to fill prescriptions (up to a 90-day supply) by any of these methods:

HealthTrust partners with Northeast Delta Dental to offer a variety of dental coverage plans, which promote good oral health, preventive dental habits, and peace of mind about having access to often costly dental procedures.

Dental Coverage Documents

The primary documents provided to Enrollees explaining benefits associated with dental plan coverage through HealthTrust include the Dental Plan Description and the Outline of Benefits.  

The Dental Plan Description explains the conditions of coverage and benefits under the dental plans employers offer through HealthTrust. Individuals covered under a HealthTrust dental plan should review this document carefully in order to understand how to get the maximum benefit of coverage provided under the plan.

The Outline of Benefits describes the specific benefits associated with the dental plans offered. An Outline of Benefits is provided to the Enrollee upon initial enrollment into the plan and upon plan updates.

These documents are available in the Secure Enrollee Portal to individuals covered by a HealthTrust dental plan. Covered individuals may also obtain a paper copy of the documents by contacting HealthTrust Enrollee Services at 800.527.5001 or enrolleeservices@healthtrustnh.org.

Domestic Partner Riders are also available to Member Groups offering coverage for Domestic Partners. Individuals covered under the plan can contact their employers regarding which, if any, Domestic Partner Rider is offered and find the applicable Rider (if their employer offers one) in the Secure Enrollee Portal.

Vision and Hearing Discount Program

Routine vision and hearing care is an important part of your overall health. HealthTrust partners with Northeast Delta Dental to offer a discount program on vision and hearing products and services to Enrollees and Retirees with HealthTrust dental coverage and their family members. Read the flyer: Delta Dental Vision and Hearing Discount Program

Eligible HealthTrust participants can enjoy reduced fees for eye exams, contact lenses, eyeglass frames and lenses, and more. The EyeMed Vision Care discount program offered through Northeast Delta Dental includes a schedule of benefits with varying discounts for specific vision care services. These discounts are available at thousands of eye care providers nationwide including private practicing optometrists, ophthalmologists, opticians, and leading optical retailers such as Target Optical, LensCrafters, and Pearle Vision. 

Hearing Discounts

Hearing loss affects 1 in 7 Americans but 95 percent of hearing loss, particularly sensorineural types caused by aging or noise exposure, can benefit from or be treated with hearing aids. The Delta Hearing Care Program offered through Amplifon includes discounted, set pricing on thousands of hearing aids and 40% off hearing exams at convenient locations.

Our Benefit Advantage Flexible Spending Accounts (FSAs) and Health Reimbursement Arrangements (HRAs) can help our Enrollees meet their healthcare and dependent care costs while providing tax-saving benefits.

Disability and Life

HealthTrust directly administers our short-term disability program and partners with National Insurance Services/Madison National Life Insurance Company to provide long-term disability and life coverage plans. These plans offer valuable benefits to Enrollees at low costs to employers.

Disability

When serious illness or an accident strikes, financial considerations can extend well beyond medical coverage. Disability insurance provides valuable financial protection to help disabled Enrollees pay for day-to-day living expenses that medical insurance does not cover, such as mortgage, rent, utilities, or groceries. With comprehensive coverage that encourages and supports recovery, Enrollees covered under HealthTrust disability programs and their employers benefit from a positive focus on the employee's return to work and a productive lifestyle. Read more: Short-Term Disability and Long-Term Disability Promotional Booklet

HealthTrust Short-Term Disability

HealthTrust's short-term disability program is designed for individuals who become disabled through illness or accident while away from work and provides benefits for a 13- or 26-week period. It is administered directly by HealthTrust and designed to work seamlessly with HealthTrust's long-term disability program.

Short-Term Disability Coverage Documents

The primary documents explaining benefits associated with short-term disability plan coverage through HealthTrust include the Short-Term Disability Plan Summary and the Schedule of Benefits.

The Short-Term Disability Plan Summary describes the benefits, terms, and conditions of the short-term disability plans offered through HealthTrust.

The Schedule of Benefits describes the specific benefits associated with an employer’s short-term disability program and is provided directly to covered Enrollees upon initial enrollment and upon plan updates. Enrollees and Member Groups currently participating in HealthTrust’s short-term disability coverage can request copies of the Schedule of Benefits by contacting HealthTrust Enrollee Services at 800.527.5001 or enrolleeservices@healthtrustnh.org.

Individuals covered under a HealthTrust short-term disability plan should carefully review these documents to understand how coverage is provided under the plan.

HealthTrust Long-Term Disability

HealthTrust’s long-term disability plans, provided in partnership with National Insurance Services and Madison National Life Insurance Company, can pick up where short-term disability leaves off or it can be offered on its own. Becoming disabled for longer than six months can have serious financial implications for any individual or family. While some individuals can tap into their savings to get by without working, few people can afford to stop working altogether for an extended period of time.

Life

HealthTrust's life coverage plans offer customizable benefits with comprehensive coverage. Life coverage is provided in partnership with National Insurance Services and Madison National Life Insurance Company. As with all HealthTrust benefit offerings, life coverage programs are provided with outstanding customer support and a commitment to service excellence.

HealthTrust Secure Enrollee Portal (SEP)

Our Secure Enrollee Portal (SEP) is the one stop, go-to resource for information about your HealthTrust coverage, programs, and resources. HealthTrust Enrollees and covered family members age 18 and older can each create a secure online account to access resources to get and stay healthy. We recommend you create an SEP account to access your coverage documents and IDs, well-being benefits, and educational resources.

For Enrollees and Retirees

The Retiree Education Center offers advice and resources for individuals at different stages of their life, such as Retiring and Under 65, Early Retiree Turning 65, and Retiring and Over 65.
