Please enter your name in the appropriate fields. Do not use any punctuation marks other than apostrophes and hyphens that may occur within the spelling of your name.
Example 1: "Renée" should be entered as "Renee" (without the accent above the "e").
Example 2: "O'Connor" should be entered as "O'Connor" (with the apostrophe).
Example 3: If you have a suffix, like ", Jr.", it should be entered after your last name as "Jr" (without the comma or the period).
